Instructions:

  • Current APU Employees (full-time, part-time and temporary staff, faculty including adjuncts) must change their names through the Office of Human Resources.  Please contact HR for assistance.

  • Legal Name change requests must be accompanied by a photocopy of official documentation.  Official documentation is a marriage certificate, birth certificate, social security card, driver's license, or passport.  Preferred Name change requests do not require official documentation but are subject to Registrar review. 

  • Address, phone, FERPA authorization, and email changes can also be made at home.apu.edu > My Links > My Information Menu.

  • Social Security Number updates must be accompanied by a copy of your Social Security Card or documentation from the Social Security Administration.
